RULES FOR USING GIFT CARD (Excerpt from the Regulations)

• A Gift Card is valid for a specified amount or entitles you to use the services specified on it at Klinika Ambroziak.
• A Gift Cardd is personal, only the person to whom the Card was issued is entitled to use it.
• Expiration date of Gift Card is one year from the date of issue, without the possibility of renewing it.
• In order to arrange a date of a visit or surgery, please contact the call-center by phone: +48221115005, +48726101000 or by e-mail: recepcja@klinikaambroziak.pl.
• When registering, please inform call-center that the form of payment for the treatment will be the Gift Card. Please show your Card at the reception desk before using the treatment.
• A Gift Card is valid only with the signature of Joanna or Marcin Ambroziak.
• Settlement with a treatment card cannot be combined with promotions and discounts.
• A Gift Card can be used for one or more visits depending on the services or values ​​specified.
• A Gift ard cannot be returned or exchanged for cash. It is also not possible to spend the rest on Gift Card.
• A Gift Card for a specific amount can be combined with other payment methods.
• The services indicated on a Gift Card cannot be exchanged for other services offered by Klinika Ambroziak.
• A Gift Card can be purchased directly at the Clinic's reception desk or ordered by e-mail: recepcja@klinikaambroziak.pl.
• For an additional fee, at the request of the Buyer, a Gift Card can be sent to the address indicated.
